For a SUPA Club to operate in a school, a number of forms need to be completed. These are:
Event Approval Forms Every event which bears the SU logo e.g. camps, missions, school groups, need to be registered with the movement. This provides, among other benefits, insurance and public liability cover. This form needs to be completed every year for the life of the ministry event.
Notification Form
This form needs to be completed by a Chaplain who is planning on establishing a SUPA Club in their school.
SU Event Form
This form needs to be completed where a Chaplain is not at a school and where a local church or group of volunteers is planning on establishing a SUPA Club.
SU Volunteer Application Form Working with Children Check The Commission for Children and Young People Act 2000 (Qld) requires ALL volunteers working with children on a regular basis to undergo a criminal history check. For SU this will apply to ALL leaders on camps, missions and school based clubs. A Blue Card is sent from the Commission to successful applicants and is valid for 2 years. Your nearest SU Schools? Worker or Regional Coordinator needs to sight this card or sign your Application Form for a card. It is a free service for all volunteers.
